Introduction: Neuralgic amyotrophy is considered to be a form of mononeuropathy multiplex which generally involves the nerves of the upper limbs.
Observations: We describe four cases of neuralgic amyotrophy revealed by dyspnea due to phrenic nerve involvement, with asymptomatic motor deficit in the upper part of the brachial plexus.
Conclusion: Phrenic nerve involvement revealed by dyspnea is rare. Associated with neurologic signs, the diagnosis is evocative. In case of an isolated attack, the key to diagnosis is the electromyographic exam.
