Mutations in the BRCA1/2 genes increase the risk of breast and ovarian cancer, leading to complex medical decisions (surveillance, risk-reducing surgery) and major psychological consequences. A support group for BRCA1/2 mutation carriers has been conducted at the CHUV since 2016 by a psychologist-psychotherapist, a genetic counselor, and a geneticist. This article presents the context and then develops some of the psychological issues raised by the participants, focusing in particular on the notions of risk, uncertainty, and guilt. The processes at work in the group, between identification and differentiation, allow for a valuable work of appropriation and subjectivation around the presence of this genetic mutation.
Les mutations dans les gènes BRCA1/2 augmentent le risque de cancer du sein et de l’ovaire, induisant des choix médicaux complexes (surveillance, chirurgie de réduction de risque) et des conséquences psychologiques majeures. Un groupe de parole dédié aux femmes porteuses de mutation BRCA1/2 est mené au CHUV depuis 2016 par une psychologue-psychothérapeute, un conseiller en génétique et une généticienne. Cet article présente le contexte, puis développe certains des enjeux psychologiques présents chez les participantes, qui se déploient notamment autour des notions de risque, d’incertitude et de culpabilité. Les processus à l’œuvre dans le groupe, entre identification et différenciation, permettent un travail d’appropriation et de subjectivation autour de la présence de cette mutation génétique.
